SENIOR CARE ADVOCACY
The
 Senior
Care Advocate program provides case
management with compassion as the Foundation’s Care
Advocates meet with clients on a one-on-one basis for
support, resources, and aging concerns. The Senior Care
Advocate program, completely dependent on donations,
grants, and bequests, offers clients the convenience and
comfort to meet in the senior’s home. After completing
an assessment for each client’s needs, the Senior Care
Advocate is able to develop a care plan that will
provide and recommend resources and services pertinent
to each case.
The Senior Care Advocate program
works with many local organizations and government
offices to ensure the highest quality of service and
care. Organizations include: Fallbrook Volunteer Senior
Patrol/YANA (You Are Not Alone), Adult Protective
Services, American Red Cross, Lifeline, People to
People, and many more.
